This form allows anyone who is a member of staff at any site where South Coast Medical Group is contracted to deliver NHS services to anonymously report inappropriate workplace behaviour. Inappropriate behaviour is anything that constitutes harassment, bullying, discrimination and sexual misconduct.
This form should be used to raise concerns resulting from the behaviour of third parties including staff, patients, students, trainees, or contractors.
South Coast Medical Group takes this type of complaint very seriously. There are several support and reporting options available for those that have experienced any type of harassment. However, South Coast Medical Group understands that formally reporting an incident is not something that everyone wishes to do; it takes time and energy, and sometimes includes sharing very personal information.
Therefore, this option enables you to anonymously record this type of behaviour. The information you provide will be collated and analysed to help monitor the prevalence of incidents that take place and allow South Coast Medical Group to consider whether further steps might be necessary to reduce or prevent inappropriate behaviour wherever possible.

Formal reporting options
Please note South Coast Medical Group cannot take direct action as a result of anonymous reporting. If you wish action to be taken you can formally report an incident to your line manager, a Partner, Human Resources or the Police.
