Use this service to register with a South Coast Medical Group Surgery. We have 5 Surgeries covering most of the Bournemouth and Christchurch area. You may wish to choose the Surgery closest to your home address.
Information:
We provide a home-visit service to housebound patients who live within their Surgery’s practice boundary. If you live outside our boundary, we may still be able to offer you a restricted set of services if you are able to travel, but we aren’t able to offer you home-visits.

Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
You can also visit the Surgery you wish to register with and pick up a registration pack in person.
