Our records show that you recently missed a scheduled appointment. If there has been any confusion or if unforeseen circumstances led to this, we kindly ask you to contact us at 01202 395195 so that we can address the issue.
This is the third instance within the past 12 months where a scheduled appointment has been missed. As outlined in our “Did Not Attend” (DNA) policy, a maximum of three missed appointments within a 12-month period may lead to a review of your registration with the practice, which could result in restrictions on future bookings or, in some cases, removal from our practice list.
Should another appointment be missed within the same 12-month period, we will need to take further action, which may include restrictions on future appointments or the potential removal from our practice list.
To help ensure appointment availability for all our patients, we would greatly appreciate at least 24 hours’ notice if you are unable to attend your appointment.
