How to Change Your Personal Details
- Use the NHS App or SystmOnline
- Print out a Change of Personal Details Form and bring it in to the Surgery. The form is also available to collect from your Surgery’s reception.
Change of Address
If you have moved to a new address, you will need to be within your Surgery’s practice boundary to be eligible for our home-visit service. If you move out of your practice boundary, you may wish to register at one of our other sites or find your nearest GP.
Changing Your Number or Email Address
You will need to show us identification before we can change your number or email address.
Changing Your Name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
